The Gymcatch system will allow you to either upload a document or add a link to an online questionnaire within Participation questionnaire section of the Participation information bolt-on/feature, not both.
However, If you wanted, you could add the document in Participation information and a link to the online questionnaire in the Business details section. The link would then appear on your Contact & Info sections of your business profile.
Please note that any document uploaded or linked added to the Participation questionnaire section is not a mandatory step of the booking flow due to it being externally completed. We suggest putting any mandatory information within the Participation waiver for your customers to agree to.
