The Gymcatch system will allow you to either upload a document or add a link to an online questionnaire within the Participation information bolt-on/feature, not both.
However, If you wanted, you could add the document in Participation information and a link to the online questionnaire in the Business details section. The link would then appear on your Contact and Info sections of your business profile.
